Position Overview:

As a Home Health Aide, you will provide personal care, household assistance, and support with daily living activities that enhance our patients' quality of life, allowing them to remain comfortably at home while maintaining confidentiality and professional boundaries.

Essential Job Functions:

  • Deliver services as ordered by the physician in the plan of care, adhering to state laws.
  • Perform, assist, evaluate, and document medical care, including:
    • Basic First Aid
    • Recording vital signs (temperature, pulse, respiration)
    • Range of motion exercises
    • Ambulation, transfer, and positioning with medical equipment
    • Meal preparation according to dietary needs
    • Personal hygiene assistance (bathing, grooming, dressing, etc.)
    • Household management (safety, sanitation, light housekeeping)
    • Thorough documentation of patient/family and company records after each visit
    • Report significant changes in patient’s condition to the RN Nursing Supervisor

Requirements:

  • High school diploma or GED
  • Current CPR certification
  • Valid Driver’s License and Acceptable MVR
  • Ongoing Education: Minimum of 12 hours of in-service training annually

Physical Requirements:

  • Ability to speak, write, read, and understand English
  • Must be able to travel
  • Ability to lift up to 50 pounds
  • Prolonged walking, standing, bending, kneeling, reaching, and twisting
  • Ability to sit and climb stairs
  • Visual and hearing acuity
  • Strong sense of smell and touch
  • Ability to reposition patients and move equipment without assistance

Must be able to respond appropriately to emergency situations in the home or during transport.

Environment:

Must be able to function in various environments, which may involve exposure to allergens and other conditions, including potential exposure to blood, bodily fluids, and infectious diseases.
